If you have previously completed the questionnaire you may wish to do so again, particularly if you have a relevant medical condition and/or high BMI (>/= 30).
The questionnaire, which is voluntary, is intended to help you identify your vulnerability to Covid-19 using a medical risk assessment developed by the Association of Local Authority Medical Advisors (ALAMA) andn forms part of our Covid-Safe arrangements.
The questionnaire allows you to assess your vulnerability to Covid-19 based on the main risk factors associated with the disease (age, sex, ethnicity, BMI and medical conditions). We are using the questionnaire as a tool to tailor support to individuals who may have a higher Covid vulnerability.

At the end of the questionnaire, you will have the option to share the results with your line manager. If your vulnerability score is 'very high' or 'high' you should share your results so that you can have an informed discussion about the most appropriate way to help you to continue to work safely.
The student version of this survey has also been updated to reflect the changes in methodology and will be reshared with students.
